Why am I exhausted when my thyroid labs are normal?
If your thyroid labs read normal but you are still cold, tired, and stuck, one reason worth checking sits a step past the gland. Your thyroid mostly releases the storage hormone T4, and your body has to convert that into the active hormone T3. A standard panel checks TSH, not that conversion.
Why can a normal TSH sit on top of a cold, tired body?
Most of your active thyroid hormone is not what your gland releases. The gland mostly puts out the storage form, T4, and your liver and tissues convert that into the active form, T3, in a second step.
That conversion is its own private number. It depends on cofactors, on how much stress chemistry is circulating, and on the raw material your diet supplies, and it does not show up on the one marker a standard panel checks. So your TSH can read 2.4, comfortably inside range, while the hormone that decides how warm you run is not landing in your tissues.
What is T4-to-T3 conversion, and why does it decide how warm you run?
T4 is the storage form, and T3 is the active form your cells use to set the pace of your metabolism. The conversion from one to the other runs on the deiodinase enzymes, and it is sensitive to the state of the rest of your body.
When stress chemistry is high or the raw material is short, that second step slows, and less active hormone reaches your cells. Your labs still read the storage form as present, so on paper you look fine, while the machinery that spends the hormone is running cold.
This is why one root problem can show up as cold hands and feet, an afternoon energy crash, more hair shedding than usual, a gut that moves slowly, and waking up unrested. These can look like six separate issues. They are often one problem wearing six faces.
What can you actually do about it?
The general direction is to take the stress load off the conversion step and supply the raw material it needs. In practice that often begins with feeding the step: a salted carbohydrate at your first meal and at your last, alongside the specific cofactors a given body tends to be short on.
The specifics are individual. Which cofactors, which foods, and in what order depend on your case, your history, and your constraints. The direction here is the mechanism. The exact protocol is where a read of your own numbers comes in.
How would you know it is working?
Waking temperature is a cheap and honest read. A warm metabolism tends to sit around 36.6 to 36.8 degrees Celsius on waking. If the conversion step is the right lever for your body, your waking temperature should begin climbing toward that range over two to three weeks, and the afternoon crash usually softens first.
In one case a reader's waking temperature climbed from 35.8 to 36.2 degrees within a few weeks, with the warmth tracking alongside higher motivation and steadier energy. If nothing moves at all after about four weeks, the conversion read was not the right one for that body, and the sensible step is to change course.
When should you see a doctor?
